I hope this note finds you well, and thank you again for the time, care, and heart you bring to the life story work – it truly makes a difference.
I’m writing to gently remind everyone about the Life Stories Program Feedback Survey, which we introduced around six months ago. So far, we've only received a small number of responses, and while those have been incredibly valuable, we’d really appreciate more input to help shape the future of the program.
The survey is designed for clients (or their families) to complete once they’ve received their final story.
Attached is a copy of the survey form.
Whenever possible, we ask that this form be offered at the time of handover – ideally, completed on the spot if the client is comfortable. If that isn’t convenient, you can leave the form with the client or family along with a self-addressed, postage-paid envelope, so they can return it directly to us by post or email.
I have a supply of envelopes ready to go – just let me know if you’d like some posted out to you.
